Dr. Imran Haque, MD- Chief Medical Officer/Founder
Dr. Imran Haque is an internal medicine physician with offices located in Asheboro and High Point North Carolina. He grew up locally in Greensboro, NC. He received his undergraduate degree from North Carolina State University and went on to medical school at Iberoamerican Medical University. He completed his internal medicine training at the University of Virginia Internal Medicine program at Roanoke-Salem. He has been practicing locally for nearly 20 years. Dr. Haque has served as Chief of Medicine at local hospitals and is currently medical director of several skilled nursing facilities, assisted livings, group homes, and hospice. Dr. Haque prides himself in providing compassionate and attentive care to all of us patients.
Robert George, MSN
Robert is a caring and patient-centered Nurse Practitioner with over 26 years of prehospital emergency medicine, to include air medical, emergency room, and ICU nursing experience. He provides up-to-date and evidence-based comprehensive internal medicine to patients in and around Randolph County. His focus is in chronic disease management to include hypertension, diabetes, COPD, weight loss, and other chronic medical conditions. Robert can also complete sport, school, work, and Commercial physicals.
Jennifer Linney RN, FNP-BC, ACAGNP-BC
Jennifer graduated from the University of South Alabama, emergency medicine, dual enrollment program. She is a Family nurse practitioner and Acute care adult Gerontological nurse practitioner. She holds certifications in sexual assault nurse examiner and forensic nurse examiner. Jennifer is originally from Detroit and moved to the Charlotte area in 2006. She met her wonderful husband and then convinced her family to move here as well. She and her husband have fur babies, an old lady cat Peanut, a pocket bully girl Paisley and a pug/Pekingese boy Biscuit. They rescued all of them and will continue to do the same in the future. Jennifer loves being a medical professional, there is nothing more rewarding or satisfying at the end of the day, then the difference she feels she made.
April Bland, MSN/Ed, APRN, FNP-C
April received her BS/BSN in nursing and Psychology from Bowie State University. She then went on to receive her post-master’s certificate from Purdue Global University and has been a Nurse Practitioner for nearly 2 years. April is a Maryland native (DMV) but Charlotte, NC has been home since 2017. She has been a nurse for 8 years specializing in women’s and children care (L&D, High-Risk ICU/OB, Postpartum, and Antenatal). April continued to develop her knowledge as a professional with years of work with the frail elderly population and adults in acute care (hospital) settings from cardiac observation, general medical/surgical, and oncology. She is a mom to a 12-year-old, 7th grader and two fur pups (Teany and Coco). She is the baby of three sisters. April loves traveling and enjoying time with family and friends.
April has always had a passion to care for people of all ages. She chose geriatric care because it provides her the comfort of knowing she is caring for those vulnerable and in need of truly passionate care. They have lived their lives and now is time for them to rest and be taken well care of. She thoroughly enjoys spending time getting to know her patients individually and building the unique bond to be able to provide the best care.
Jennifer Hagan, AGPCNP
Jennifer is an Adult-Geriatric Primary Care Nurse Practitioner, board certified by the American Association of Nurse Practitioners. She graduated from Western Governors University with a Bachelor of Science in Nursing in 2016 and from South University in 2018 with a Master of Science in Nursing. Jennifer began her career in nursing in 2013 as a floor nurse with experience in cardiac, step-down, and intensive care units. She now works as a nurse practitioner in long-term care, since beginning her nurse practitioner career.
Jennifer grew up in Asheboro, North Carolina. She has always loved working with the geriatric population and enjoys a career that allows her to do just that. She currently cares for patients at skilled nursing facilities, as well as assisted living facilities providing a wide range of health care needs, from health maintenance, chronic care management, acute care management, and end of life care.
Riley Eudy, NP
Riley is a Family Nurse Practitioner certified by the American Academy of Nurse Practitioners. She earned her Associate’s Degree in Nursing at Southeastern Community College in 2009. She then went on to complete her Bachelor of Science Degree in Nursing at East Carolina University in 2012, and most recently received her Master of Science Degree in Nursing at The University of North Carolina at Charlotte in 2022. The majority of her 14 years in nursing have been in cardiovascular and intensive care specialties. She believes in providing holistic, patient-centered care by using evidence-based practice as a guide to promote health and wellness throughout the lifespan. Riley is passionate about geriatric medicine as she feels that this population offers a unique opportunity to build meaningful bonds. She believes in empowering each patient by involving them in their own plan of care to help them identify and achieve their healthcare goals. In her personal time, Riley enjoys fishing, long-distance running, and spending time with her husband and two children at their farm in Peachland, NC.

Andrea is a board-certified Adult/Geriatric Primary Care Nurse Practitioner by the American Academy of Nurse Practitioners. She has a Masters of Science in Nursing (MSN) from the University of North Carolina at Greensboro and a Bachelor of Science in Nursing (BSN) from the University of North Carolina at Chapel Hill.
After a decade of working as a nurse, primarily in critical care settings, she chose to pursue a career as a Nurse Practitioner. Andrea prides herself in delivering personalized, holistic care to each patient. Andrea believes that the best way to provide high-quality patient care is to acknowledge each patient’s individuality and meet them at the point of their need. Care should be patient-focused with the patient at the center of decision-making. Health, wellness, and illness should be viewed through the lens of the patient as a whole. Patient care should be holistic and compassionate with an emphasis on educating the patient while building a strong therapeutic relationship built on trust and best evidenced-based practice. Andrea enjoys all aspects of family medicine but she has a special love for geriatric patients. NC Elder Care is lucky to have Andrea returning to provide care for its residents.

Dana Lee Hines is a dedicated healthcare professional with over 20 years of experience as a Registered Nurse and 7 years as an Advanced Practice Registered Nurse. She is credentialed by the American Association of Nurse Practitioners and is an active member of the North Carolina Nurses Association, the American Nurses Association, and the Gerontological Advanced Practice Nurses Association.
Dana’s expertise lies in Long Term Care, Acute Care, and Chronic Care Management, where she has consistently provided compassionate and comprehensive care to her patients.
She earned her Bachelor of Science in Nursing from North Carolina Agricultural and Technical State University in 2000 and her Master of Science in Nursing from South University in 2017.
On a personal note, Dana is happily married and the proud mother of one daughter and two bonus children. She is also a grandmother to three wonderful grandsons. In her free time, she enjoys line dancing, ballroom dancing, and exploring new destinations—her favorite being the beautiful island of St. Lucia.

Jamie Graham is a dedicated Family Nurse Practitioner with over 17 years of experience in the medical field. Her background spans various areas of healthcare, including emergency and urgent care, family practice, skilled nursing facilities, and hospice care.
Jamie holds a Bachelor’s degree from UNC Charlotte and a Master’s degree from Duke University.
When she is not caring for patients, Jamie enjoys spending quality time with her family and dogs. She also loves heading to the beach, hiking, and taking her jeep out for a drive.
Catherine Conrad, FNP-C
Catherine Conrad, FNP-C, is a board-certified Family Nurse Practitioner with an extensive work history in healthcare spanning over 17 years. Her experience includes MedSurg/Telemetry, General Surgery/Trauma, Behavioral Health, Geriatric Behavioral Health, Home Health, LTC/SNF, and Post-Acute Care/Rehab. Catherine is particularly passionate about working with the geriatric population, with primary interests in dementia care, maintaining patient dignity, advanced care planning, and end-of-life care.
She graduated from Winston-Salem State University and has been practicing as a provider since 2022.
On a personal note, Catherine is a devoted dog mom to two large dogs, a 150-pound Great Pyrenees and a 90-pound Chocolate Lab Retriever. She spends most of her free time with them, enjoying car rides and short walks. Catherine is also fascinated by Greek Mythology, the Roman Empire, Vikings, and Medieval times.
Lauren Haas, AGPCNP-C
Lauren Haas, AGPCNP-C, is an Adult Gerontology Primary Care Nurse Practitioner who recently graduated and is beginning her Nurse Practitioner career with NC Elder Care. She has 5 years of experience as a registered nurse, specializing in ICU care with a focus on neurosurgery and Covid-19, as well as geriatric primary care. Lauren pursued a career in geriatrics during graduate school, knowing she wanted to continue this path as an NP.
Lauren graduated Magna Cum Laude from the University of South Carolina with her BSN in 2019. She earned CCRN and SCRN certifications while working in critical care. She then graduated Summa Cum Laude from Duke University with her MSN in the spring of 2024.
Originally from Louisville, Kentucky, Lauren now resides in Charlotte with her husband, Chris, whom she married in April 2024, and their golden retriever, Crouton. In her free time, she enjoys traveling, reading, running, and attending college football and basketball games with her husband and friends.

Dr. Dana E. White, Ed.D, MA, LCAS-A, is a graduate of UNC-Charlotte with a Bachelor of Social Work and Guilford School of Theology with a Master of Arts in Counseling Psychology. She also holds a Doctorate of Education in Counseling Psychology from Guilford School of Theology.
Dr. White’s clients are adults who may be experiencing challenges with addiction, which may manifest through anxiety, mood disorders, life transitions, trauma, grief, and loss. She firmly supports active listening as an antidote to the impersonality of our increasingly hectic lives. Dr. White’s counseling style is a collaboration of strength-based approaches, coupled with helping people recognize and utilize their inner wisdom and resources. She believes that every person has a story and is fascinated by hearing about their journey through challenges, interests, struggles, and achievements. Dr. White is dedicated to helping people alter past patterns that may not have been productive and guiding them toward success. She specializes in the treatment of substance abuse disorders and welcomes your call to begin the emotional healing process.
